Children under the age of 19 will qualify for the “additional Child Premium”. If the child is a full-time college student, then they are covered until the age of 23.There is no ID card, group number, or membership number. All your membership information will be kept on file within our office.Your effective date is the day you sign up, and your renewal date is the same each year. There is no waiting period or missing tooth clause.No refunds are given if a patient does not use their benefits.Benefit Summary
